500 Cities MapsThe 500 Cities Project provides city- and census tract-level small area estimates for the largest 500 Cities in the United States. These data are derived from the Behavioral Risk Factor Surveillance System (BRFSS) and have been incorporated into interactive and static maps. Measures for the 500 Cities project include chronic disease risk factors, health outcomes, and clinical preventive service use. The 500 Cities project is a collaboration between the Centers for Disease Control and Prevention (CDC), the Robert Wood Johnson Foundation, and the CDC Foundation.
