Atlas Plus: Social Determinants of Health County and State Maps

Atlas Plus MapsThe American Community Survey is an ongoing survey, facilitated by the United States Census Bureau, that provides vital population information on a yearly basis about the nation. The data from this survey is used by the Centers for Disease Control and Prevention to create static maps that highlight several Social Determinants of Health at a county and state level. The following areas are highlighted in these maps: poverty rates, median household income, population 25 and older without a high school diploma, vacancy of housing units and rate of primary care physicians per area.
